Group Instruction and In-Service Form Completion

Overview

The Group Instruction / In-service form was developed to assist trainers and communities with a comprehensive tool to collect individual employee data and course materials. This instruction will give the users the overview of the form functions and how to work with the fields.   


Selecting your Community and entering the Date/Time

From the dropdown menu, select your community oe enter your community name. Enter the start and end date of the Group Instruction / In-Service. And the Start and End Time of the instruction. 


Enter the Instructor Details

Ensure that the person Leading or Giving the Group Instruction / In-Service completes the Instructor / Proctor Section. 

 

The First, Lastname, and email is required to submit the form


Entering Course Detail

1. Enter the Course Number and Course Name and if there is an associates skills test with the course of instruction.

 

2. To add additional courses, select the "ADD COURSE" button and enter the information in. 


Adding Employee Detail / Roster

1. Enter the employee First Name and Last Name. 

 

2. If you are capturing employee signatures, select Yes. otherwise select no. When selecting yes, The instructor can capture the employee signature electronically. this will embed the signature in the form and support signature verification for training and in-service validation and verification. 

3. To add Additional Employees, select the "ADD EMPLOYEE" button. 


Uploading Supporting Documentation.

Courses that require additional documentation or skills checks, can upload them directly into this form. this allows for consolidation and archiving of Group Instruction and In-Service related activities.

1. To upload documents, scan or take pictures of the supporting documentation. Once captured, select the "UPLOAD" button to begin the transfer. 

 

Instructor / Proctore Confirmation 

Once the Group Instruction / In-Service is completed, the Instructor / Proctor can digitally sign the form to capture their signature. 

 

Saving the Form

For Group instruction / In-Services that take more than one day, or instructor that pre-fill the form out, the form can be saved for access and editing. 

 

To save the form, select the "SAVE" button.

Pressing the save button will generate a unique URL that the instructor can access at a later time to complete.

The instructor should complete the email me my link to recieve the URL. Once entered, select "SEND" and an email will be sent to the address. 

IMPORTANT: Once the save button is selected, this form can only be accessed using the URL provided in the email address or by contacting Support@felsandassociates.com

 

Submitting the Form

 

Once the Group Instruction / In-Service is completed, and all signatures and documentation has been uploaded or captured, select the "SUBMIT" button to complete the form. 

All information will be send to the email address associated with instructor or administrator. The final output for the report will look similar to the following pdf.
